CHS Outreach Services coordinated and facilitated a health talk on healthy nutrition for seniors who required additional information and education on sound nutrition practices, necessary for them to maintain a healthy lifestyle. Lauderdale Lakes Health Center and Cora E Braynon Family Health Center were positioned as access points for primary care and wrap around services for seniors in need of a medical home.
